  • 1- هذا النموذج مخصص لتسجيل بيانات الحالات، ولا يعتبر التسجيل في هذه الاستمارة قبولاً للطلب.
  • 2- يرجى تحري الدقة في إدخال البيانات وفي حال عدم تطابق البيانات المدخلة مع المستندات سيتم رفض الطلب
  • 3- سيتم فرز الحالات من قبل البرنامج والتواصل مع الأكثر استحقاقا.
  • 4- تسجيل البيانات متاح لقاطني إمارة دبي، عجمان، ورأس الخيمة
  • 5- يمكن تعبئة الاستمارة بشكل مباشر من قبل مقدم الطلب علماً أن الجمعية ليس لها تعاقد مع أي مكتب من مكاتب الطباعة.
  • 6- ضرورة إكمال جميع المستندات المطلوبة في الرابط حسب الترتيب ووضعها في ملف pdf واحد وإرفاقها في نهاية الاستمارة (يرجى الضغط على الرابط لمعرفة المستندات المطلوبة)
  • 7- الحد الأقصى لحجم ملف pdf يجب ألا يزيد عن 10 ميغابايت .

    • 1- This form is intended for the registration of case data, and registration in this form is not considered an acceptance of the application.
    • 2- Please check the accuracy in entering the data and in case the entered data does not match the documents, the application will be rejected
    • 3- The cases will be sorted by the program and communicate with the outstanding cases only.
    • 4- Data registration is available for residents of Dubai, Ajman, and Ras Al Khaimah.
    • 5- The form can be filled in directly by the applicant, noting that the association does not have a contract with any of the printing offices.
    • 6- The need to complete all the required documents in the link in order, put them in one pdf file and attach them at the end of the form (please click on the link to find out the required documents)
    • 7- Maximum PDF file size should not exceed 10MB.
